Small Business Agency Cooperative Inc. (SB/A CoOp) group health care plans are the perfect comprehensive, affordable, and partially self-funded plans offering level funding. The healthcare coverage programs are federally regulated under ERISA and establish the standards for over 60% of employer-established healthcare plans in the private and public sectors.

Small Business Employers can now attract and retain valuable employees with a robust medical benefits plan including Preferred Provider Organization PHCS, Serve You Rx pharmacy benefit manager, and Teledoc Health. Affordable Care Act qualified, you can be assured of quality care to meet employee benefit needs and manage costs.

With a Loomis TPA partnership, you can expect seamless customer service for all claimants. As a Third Party Administrator, Loomis has been simplifying the claim process for over 60 years.

SB/A CoOp offers Employer health care benefit programs for 3 or more enrolled. Employers can include W2s working at least 16 hours per week or 1099 contractors under an ACH list bill. A variety of plans are available such as traditional plans with annual coverage limits, and economical health care plans utilizing co-pays and hospital daily rates. Enrolled employees may add a dental/vision plan or even voluntary benefits.

All of these benefit plans feature our same philosophy of affordable cost, comprehensive coverage, and easy-to-understand coverage provisions.  Our enhancements include:

  • Five “SB/A Core Health Plans” with Annual Maximums ranging from $10,000 to $250,000 per member
  • Three “Freedom ICON Plans” that cover primary and emergency care services and generic prescriptions
  • The “SB/A MEC Plan (Minimum Essential Coverage) primarily covers preventative and wellness screenings as designated by the Affordable Care Act. MEC plans create opportunities to stay on top of personal health before it might become an issue.
  • Two “SB/A Dental / Vision Plans” – one is a traditional dental design plan that directly pays DenteMax PPO network dentists and the other plan reimburses you for your dental and vision expenses from providers that you select
  • Three “Voluntary Benefit Plans” that pay you specified dollar amounts for an accident, hospitalization, or critical illness. Coming soon!

Plans include simple no-age-band pricing. No medical underwriting. Employers complete an application and a simple employee census. Both documents can be accessed on this website or call us at 888-857-2667 and we will personally answer your questions and assist you with the enrollment process.

Upon submitting the forms, Employers can expect a brief onboarding session with Loomis Customer Service Specialists on the use of the Employer Web Portal. Employers will receive an enrollment link with the menu of benefit options you have made available for Employee selection of coverage that is appropriate for their family needs.

  • Step 2 – Loomis (TPA) will set up your enrollment portal account and email logins to you
  • Step 3 – Fill out the Employee Eligibility Census
  • Step 4 – Login to your Employer portal at Loomis and securely upload your completed Census file
